  • Below, please submit a personal thank you letter to your scholarship donor. Address letters “Dear Donor:” or see your award letter for the specific name to address your letter to. Please be sure to include the answers to the questions below in your letter. Submissions should be made as a Word document. Please also include a recent photo of yourself that is appropriate to send to your scholarship donor.

    I waive my rights under the Family Educational Right and Privacy Act of 1974, as amended (Buckley Amendment), by allowing the Scholarship Office of The University of Texas at San Antonio to release information about me to the donor(s) of the above named scholarship, potential donors, newspapers.

    Failure to submit an acceptance form by the deadline noted in your award letter will result in automatic termination of the scholarship. Funds from this scholarship will not be released until we receive all information (including thank you letter and photo).
